The SWO Health Plan Stakeholders completed a Community Needs and Readiness Assessment. The top 5 included:

1. Adverse Childhood Experiences
2. Substance Use Disorders
3. Diabetes Mellitus
4. Obesity and Overweight
5. Serious Mental Illness

There are many Tribal Health programs whose aim is to help decrease these disparities. These Programs provide a wide range of services which include clinical care, substance use treatment center, health education, and prevention. In the spirit of promoting togetherness and connectivity, the SWO would like to use the Funds obtained from Dakota Magic’s first–ever Charity Golf Tournament with Hall of Famer Cris Carter towards a Gathering of Native Americans or GONA. A GONA is a culture-based planning process where community members gather to address community-identified issues.  It uses an interactive approach that empowers and supports AI/AN tribes.  The GONA approach reflects AI/AN cultural values, traditions, and spiritual practices.  This will allow all the programs to be involved and every relative encountered by each program to participate.

During the golf tournament the Asniyapi Field Health Clinic will be onsite with its mobile clinic.  The clinic has been able to deliver patient-centered primary care medicine in a variety of settings, including the home-base exam suite located in the SWO COVID-19 Isolation & Quarantine facility, and expanding to community sites where the Oyate live, reside, work, and play, including facilities with public access; worksites; those serving special populations such as substance use disorder treatment, detention, schools, and early childhood centers; homes; virtual or telehealth modalities; and a mobile (RV) venue. Primary Care services include Quick Care, Wellness Physicals, Diabetes Care (prediabetes, obesity, weight loss, and nutrition), Medication for Addiction Treatment (MAT), Wellness Physicals, Quick Care, and Tribal Employee Health.
